The article “The impact of the State Innovation Models (SIM) initiative on population models” by Partha Deb, Angelica Gangaram, and Hoda Khajavi is based on a study conducted to examine the SIM’s effects on the overall health outcomes of the states that adopted this approach. The data used in the study is obtained from individuals aged 45 years as the primary population sample from six SIM states and 15 control states. The study also involved a secondary population sampled using age, education, income, and racial status strata.  The study results indicated that the states which implemented SIM had more noticeable health reforms improvements. Also, the effects were more significant among the older population, the population from the rural areas, and those enrolled in Medicare health insurance coverage.

Measuring health and health outcomes is essential for the healthcare delivery systems and the general population. Through this, the health indicators can be determined, and best measures put in place to mitigate any lingering adverse health effects. The three critical metrics used when assessing healthier population outcomes are mortality rates, readmissions, and patient experience. These metrics intertwine in that when readmissions rates and the patient experience are lacking, the mortality rates of that population will likely be high as well. A community-based program that can alleviate health within the community is the behavioral health integration model, which incorporates behavioral and mental health into primary care. It would ensure that patient healthcare needs are met holistically. The health IT approach that would support this is the health informatics approach which entails proper storage and retrieval of patient information to ensure optimum patient-physician collaboration.

Hi David –

With the passage of the ACA, the government is seeking ways to improve healthcare and lower healthcare cost, which aligns with the goals of population health (Nash et al., 2016).   The State Innovation Model (SIM) is an initiative of the CMS that funds and encourages states to develop programs/models to address this objective.  Deb et al. (2021), examined the first six states that were selected for the SIM Program to assess success.

This week’s reading from the textbook discussed the role that information technology and various health delivery models such as those used by the states in the SIM program that can be used to achieve the goals of population health (Nash et al., 2016).

Today, we see increased use of models and frameworks to guide healthcare providers in rendering higher quality of care with a goal of contributing to better population health outcomes. For example, the Patient-Center Medical Home Model focuses on prevention and improving the delivery of primary health care using a number of strategies and Accountable Care Organizations focus on care coordination (Nash et al., 2016).
