Read each of the following scenarios. Choose 2 of the following and write responses based on what you have read about personnel, productivity, and Medicare requirements.

The hiring process seems to be working well, because you get a lot of applicants when positions open up. However, many of them quit less than a year after they are hired, some after just a few months. The pay and hours are fair and reasonable, comparable to other agencies in the area. What other factors would you look at? List at least 3 specific potential issues and how you would address them.

Productivity isn’t meeting your expectations. When you check into employee assignments, you see that the manager has been assigning employees to cases that aren’t always appropriate for their level of experience. When you talk to the manager, she tells you there aren’t enough employees to put everyone exactly where they should be. The employees tell you that the manager is disorganized, and doesn’t create the schedule far enough in advance, nor does she adequately consider who should go where. What do you do, keeping in mind that the goal is to increase productivity? Define productivity as it relates to this situation, and then explain how you would address this situation.

You are a newly hired manager to an established home health agency. Upon taking the reins, you discover that the previous manager had not followed Medicare requirements for the professional that is sent to a patient’s home or the productivity plan set up for the home health agency. What do you do first so that you can establish yourself as the manager, yet still take the time to improve morale, address negative attitudes, get to know your staff and be able to bring the agency into compliance?

This is your opportunity to demonstrate your advanced knowledge of the social determinants, primary and population health care, consultation skills, best practice standards and reflective practice. The case study must be of a child or adolescent less than fifteen years of age who is experiencing a  health problem seen in the remote setting, within the context of their family and community. 

To complete this assessment:

1.          Provide a comprehensive overview of the case which broadly considers antenatal and social environmental issues, implements best practice guidelines and primary health care concepts;

2.          Relate how the health problem(s) are connected to the overall health and well-being of the individual within his/her family and community environment; both in their early years, now and in ten years’ time

3.          Describe your assessment to demonstrate the clinical reasoning that guided your clinical consultation using the RAN model of consultation

4.          Recognise gaps in your knowledge, explaining how you addressed these gaps and discussing their impact on your RAN practice

You may like to present your case study in two parts: The first providing the information and the second demonstrating the critique. Alternatively, you may interweave both the information and the critique. In either case, it is important that you complete all components of this assessment task.
