Evidence-Based Practice and the Quadruple Aim Berwick, Nolan and Whittington (2008) allege
Evidence-Based Practice and the Quadruple Aim
Berwick, Nolan and Whittington (2008) allege that high quality care is determined by simultaneous pursuit of three aims commonly called the Triple Aim. In addition, the author highlights three overarching objectives including; improve individual experience of care; reduce healthcare cost and enhance population health. However, the Triple Aim failed to adequately appreciate the integral role of human capital in transformation of healthcare. As a result, Sikka, Morath and Leape (2015) modified the Tipple Aim to Quadruple Aim to integrate the fourth objective of enhancing the experience of care delivery. That is, staff engagement is all about fulfillment in nursing practice. The assignment highlights how evidence-based practice (EBP) can help to achieve the Quadruple Aim based on patient experience, population health, cost and work life of healthcare providers.
EBP could have an impact on Quadruple Aim in healthcare that enhances group cohesion and job fulfillment among workers (Kim et al., 2016). Job fulfilment is the fourth objective initiated by Sikka, Morath and Leape (2015) to strengthen EBP principles as well as adoption. Nonetheless, Kim et al. (2016) did not report significant relationship between EBP adoption with group cohesion and effectiveness. Nevertheless, in the study by Kim et al. (2016), a model of Advancing Research and Clinical Practice through Close Collaboration (ARCC) was utilized and found that EBP principles contributes to successful adoption of EBP that is associated with high job fulfilment and group cohesion among nursing staff. Even though EBP implementation is important in the delivery of quality care and enhanced patient outcomes, inadequate skills in EBP and organizational support hinder successful implementation. Thus, Sikka, Morath and Leape (2015) suggest that Quadruple aim can positively lead to improved patient experience, population health, and work life of healthcare providers while reducing healthcare costs. For example, the authors believe that Quadruple aim can impact EBP by allowing providers to use best evidence to save lives and minimize population suffering. The overarching goals of the Quadruple Aim are creating safe workplace for healthcare providers to enhance patient experience, population health and reduce medical costs. Similarly, Crabtree, Brennan, Davis and Coyle (2016) allege that nurses are frontline providers in healthcare and can improve patient experience using EBP. Furthermore, nurses play an important role of integrating EBP into practice. Nevertheless, Crabtree et al., (2016) share sentiments with Sikka, Morath and Leape (2015) that EPB in clinical practice can only be achieved if nurses have the necessary skills to search for best evidence, ask questions about patient care, have adequate resources and able to evaluate evidence efficacy. However, Crabtree et al. (2016) note that whilst EBP enhances clinical outcomes, the ability of Medical University of South Carolina (MUSC) to prepare nurses to use EBP is limited. In spite of this, the EBP Nurse Scholar Course has enhanced nurses’ skills as well as knowledge about practice, theory and dissemination of EBP. As a result, MUSC has been able to improve nurses’ confidence in EBP. In turn, improved confidence in EBP improves the Quadruple Aim.
By and large, based on Sikka, Morath, & Leape (2015); Crabtree, Brennan, Davis, & Coyle (2016); and Kim et al. (2016) EBP has a positive impact on the Quadruple Aim in terms of improved patient care experience, population health, work life of healthcare providers and reduced healthcare costs. In particular, EBP empowers healthcare providers contributing to high degree of job satisfaction. Moreover, while EBP reduce variation in the provision of care, it is also considered as an indicator of achieving Quadruple Aim. Nonetheless, despite the positive impact of EBP, it is not a standard of care in some healthcare organizations in US because of implementation barriers such as lack of skills, knowledge, organizational support, and misconceptions and so forth.
